When doing performance reviews I use the SHRM website. They have a paid tier but also have tons of free information and templates. It's a great starting point where you can tweak it to suit your specific needs. I think a big question would be what are you currently using and what are you looking to get out of the performance review? Is it a self evaluation style review or is it a peer review? These might help me be able to better guide you into a new more effective review questionnaire.
